Qualifikationen

• Minimum 2 years of experience with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or alternative RN license as allowed by state law.
• State-specific certification may be required by law or contract.
• May require a state-specific certification based on state law and/or contract.

Aufgaben

• Assist RN in identifying members for high risk complications and in obtaining clinical data.
• Support RN in determining members who would benefit from alternative levels of care or waiver programs.
• Provide collected information to RN for verification, interpretation, and care plan development.
• Coordinate care with members, chronic conditions, and health care team to ensure cost-effective use of benefits.
• Apply clinical guidelines and disease knowledge to assist with assessments and care planning.
