CVS Health is seeking a Case Manager Analyst - Field in Illinois to support LTSS members. This full-time, field-based position requires up to 75% travel for face-to-face member visits and hands-on care coordination, with in-person quarterly reviews and comprehensive documentation in the EHR.

The role emphasizes evaluation, care planning, and collaboration with clinicians to achieve optimal health outcomes for waiver members.

Qualifications

• Must reside in Illinois in the Chicago area zip codes listed.
• Valid Illinois Driver's license required.
• Willing and able to travel up to 75% of the time with mileage reimbursed.
• Minimum of two (2) years of case management experience.
• Microsoft Office and electronic health record experience.

Responsibilities

• Evaluate referred members' needs/eligibility and recommend an approach to case resolution.
• Identify high risk factors and coordinate with clinical case management or crisis intervention as appropriate.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers, supervisors, Medical Directors to overcome barriers and achieve outcomes.
• Escalate quality of care issues and secure appropriate options/services.
• Coach and empower members to engage in healthcare decisions with their providers.
• Ensure documentation and compliance with regulations and company policies.
