Job summary

CVS Health Corporation seeks a Field Case Management Coordinator for Knox County IL. This full-time role requires 50-75% travel and in-person member meetings to coordinate care and support their health journeys.

You will evaluate needs, develop care plans, monitor progress, and collaborate with case managers, medical directors and providers to optimize outcomes. A bachelor's or non-licensed master in behavioral health or human services is required.

Qualifications

• Minimum 2 years of experience in behavioral health, social services or human services.
• Minimum 2 years of case management experience.
• Must reside in Knox County IL (Applicable ZIP codes provided).
• Must have reliable transportation and travel up to 50–75% to meet members face to face.
• Bachelor's Degree or non-licensed master level clinician in behavioral health or human services.

Responsibilities

• Evaluate referred member’s needs/eligibility and recommend a care plan.
• Coordinate and monitor assigned care plan activities.
• Consult with case managers, Medical Directors and health programs to overcome barriers and achieve goals.
• Provide coaching/information to empower members to participate in decisions.
• Document care processes in accordance with regulatory and accreditation guidelines.
