Job summary

CVS Health is seeking a Field-based Case Manager Analyst in Illinois to coordinate care for waiver/LTSS members. You will travel to meet members, conduct in-person visits, and manage a full caseload with quarterly provider engagements.

Responsibilities include evaluating needs, coordinating care, and improving health outcomes through collaboration with clinicians and program staff. A strong emphasis on documentation, compliance, and member empowerment is required.

Qualifications

• Minimum two years of case management experience.
• Must reside in Illinois, west side of Chicago area.
• Must possess valid Illinois driver’s license and reliable transportation.

Responsibilities

• Evaluate members’ needs and eligibility and recommend case resolution.
• Coordinate and monitor care plan activities and progress.
• Collaborate with medical directors and case managers to overcome barriers and improve outcomes.
• Provide coaching and education to empower members in health decisions.
