Molina Healthcare is seeking a Field Care Manager for LTSS to conduct in-home assessments, coordinate waiver services, and develop care plans for Medicaid members in the Lancaster, TX area. This role requires travel and strong LTSS/case management experience.

Responsibilities include coordinating with caregivers, physicians, and the health care team; monitoring progress; and ensuring timely documentation. Proficiency in Microsoft Office and strong communication skills are essential.

Qualifikationen

• At least 2 years of health care experience, including LTSS and care management, or equivalent experience.
• Clinical licensure/certification required only if state contract mandates it; active/unrestricted if licensed.
• In some states, a health-care related bachelor's degree may be required.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel.

Aufgaben

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with member and care network to address needs and goals.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Promotes integration of LTSS, behavioral health care, and community resources.
• Assesses medical necessity and authorizes waiver services.
• Collaborates with ICT meetings for service approvals and coordination.
• Uses motivational interviewing to educate and motivate members.
• Addresses psycho/social, financial, and medical barriers to care.
• Identifies incidents and develops prevention plans for member welfare.
• Travels locally 25-40% as required by state/contract.
