Job summary

In this role you will determine eligible services, develop care plans, and coordinate with a multidisciplinary team to ensure quality, cost-effective member care. Responsibilities include home visits, waiver enrollment processes, and ongoing care plan monitoring.

Qualifications

• Minimum 2 years health care experience with LTSS or care management
• Proof of active unrestricted license if required by state
• Bachelor's degree may be required in some states
• Valid driver's license and reliable transportation for local travel
• Experience with Medicaid or MCO settings is preferred
• Experience with substance use disorder populations is a plus

Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including home visits
• Coordinate waiver enrollment and disenrollment processes
• Develop and implement collaborative care plans with members, families and providers
• Monitor care plans and document interventions and outcomes
• Promote service integration across behavioral health, LTSS, and community resources
• Assess medical necessity and authorize waiver services
• Facilitate interdisciplinary care team meetings for approvals/denials
• Use motivational interviewing to educate and motivate members
• Identify barriers to care and coordinate solutions
• Collaborate with licensed care managers as needed
• Travel up to 25-40% within coverage area
