Job summary

Molina Healthcare is seeking a TX licensed LVN, LBSW or LMSW to join our LTSS team as a Case Manager serving Medicaid members in the southern Dallas County area. The role requires face-to-face home visits, comprehensive assessments, and Waiver service planning; travel 25-40% may be required.

Strong MS Office skills and coordination with multidisciplinary teams are essential. Responsibilities include developing care plans, monitoring progress, coordinating services, and ensuring timely

Qualifications

• At least 2 years healthcare experience, LTSS preferred.
• 1 year of care management or equivalent experience.
• Valid and unrestricted driver's license and reliable transportation.
• Active licensure as required by state contract or regulations.
• Ability to travel 25-40% locally for member/home visits.

Responsibilities

• Completes comprehensive member assessments, including in-person home visits.
• Develops and implements care plans with waiver service plans.
• Monitors care plan effectiveness, documents interventions and outcomes.
• Coordinates LTSS with behavioral health and community resources.
• Facilitates waiver enrollment and ICT meetings and track progress.
• Uses motivational interviewing to educate and support members.
