Job summary

Molina Healthcare is seeking a TX-licensed LVN, LBSW, or LMSW to join our LTSS Team as a Case Manager serving Medicaid members in southern Dallas County. The role includes in-home visits, waiver processes, and care plan development with a focus on quality and cost-effective member care.

The position requires strong Microsoft Office proficiency, particularly Outlook, Excel, and Teams, and travel up to 25-40% as needed. Monday–Friday schedule applies.

Qualifications

• At least 2 years of health care experience, including LTSS, and 1 year in care management or related setting.
• Clinical licensure and/or certification required only if mandated by state contract or licensing boards.
• Bachelor's degree in a health care related field may be required in some states.
• Valid and unrestricted driver’s license and reliable transportation for travel.

Responsibilities

• Complete comprehensive member assessments, including in-person home visits when required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians, and health professionals.
• Monitor care plans, document interventions, and suggest changes to meet goals.
• Promote integration of services to enhance continuity of care for LTSS members.
• Assess medical necessity and authorize appropriate waiver services.
• Coordinate with the interdisciplinary care team for approvals and collaboration.
• Educate and motivate members using motivational interviewing and Molina clinical guides.
• Identify barriers to care and coordinate psycho/social and financial support.
