Job summary

Molina Healthcare is seeking a Case Manager to support Medicaid LTSS members in the southern Dallas County area. The role includes in-home assessments, care planning, and coordination with multidisciplinary teams in a Monday–Friday schedule, 8 AM–5 PM CST. Travel up to 25–40% is expected; MS Office proficiency is essential.

The position emphasizes collaboration, documentation, and ensuring high-quality, cost-effective member care with strong communication and time-management skills.

Qualifications

• At least 2 years of health care experience, including at least 1 year in LTSS and 1 year in care management.
• Clinical licensure and/or certification as required by state contract or licensing mandates.
• Bachelor’s degree in health care may be required in some states.

Responsibilities

• Complete comprehensive member assessments, including in-person home visits as required.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and team.
• Monitor care plans, document interventions and adjust as needed.
• Promote integration of LTSS, behavioral health, and community resources to ensure continuity of care.
• Assess medical necessity and authorize waiver services.
• Facilitate interdisciplinary care team meetings for approvals.
