Molina Healthcare Inc in Fort Worth, TX is seeking a Texas-licensed RN to join as Field Care Manager for LTSS. You will perform assessments to determine eligible services and coordinate waiver enrollment and care planning for Medicaid members in the local area.

This role requires in-person home visits, collaboration with an interdisciplinary team, and travel within the coverage area (approximately 25–40%). Strong Microsoft Office skills and experience with Medicaid populations are preferred.

Competenze

• Must hold Texas RN license with active/unrestricted status.
• Minimum 2 years in health care with at least 1 year in care management or LTSS.
• Valid driver’s license and reliable transportation for local travel.
• Microsoft Office proficiency (Outlook, Excel, OneNote, Teams).
• Experience with Medicaid populations within an MCO is preferred.

Mansioni

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with member, caregivers, physicians and support network.
• Monitors and documents progress toward care plan goals; suggests changes as needed.
• Promotes service integration across behavioral health, LTSS, and community resources.
• Assesses medical necessity and authorizes appropriate waiver services.
• Evaluates covered benefits and advises on funding sources.
• Convenes ICT meetings for service approvals/denials; coordinates with team.
• Uses motivational interviewing and Molina guidelines to support member change.
• Identifies barriers to care and provides care coordination for psychosocial, financial, and medical issues.
• Identifies critical incidents and develops prevention plans for member safety.

Formazione

RN license

Bachelor's degree in health care related field
