Job summary

Molina Healthcare seeks a Case Manager in the southern Dallas County area to support Medicaid members through LTSS coordination. The role involves home visits, assessments, and developing care plans with a multidisciplinary team.

Proficiency with Microsoft Office and mobility for local travel are required. Responsibilities include coordinating waiver enrollment, monitoring plans, and ensuring access to required services while maintaining high-quality, cost-effective member care.

Qualifications

• Minimum 2 years health care experience, with LTSS experience preferred.
• Licensure and/or certification required only if mandated by state contract.
• Bachelor’s degree may be required depending on state/contractual requirements.

Responsibilities

• Complete comprehensive member assessments including in-person home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and document progress.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for approval/denial of services.
• Educate and motivate members using Molina guidelines.
• Identify barriers to care and coordinate support to address psycho-social, financial, and medical issues.
• Identify critical incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25-40% locally as required.
