Job summary

Molina Healthcare in Lancaster, TX seeks a Field Care Manager, LTSS (RN) to support Medicaid members in the southern Dallas service area with in-home assessments and care coordination, including waiver services. Bilingual candidates are preferred. Hours are typical M–F, with mileage reimbursement.

Responsibilities include developing care plans, coordinating with physicians and caregivers, and ensuring timely documentation while traveling to service locations as needed.

Qualifications

• 2+ years health care experience including care management, managed care or LTSS.
• RN license active and unrestricted.
• Valid driver’s license and reliable vehicle for travel.
• Ability to work independently with minimal supervision.
• Experience with populations with disabilities or chronic conditions preferred.

Responsibilities

• Conduct in-home, face-to-face assessments with waiver Medicaid members.
• Develop and implement care plans in collaboration with members, caregivers and providers.
• Monitor and evaluate care plans; adjust as needed for outcomes.
• Coordinate with interdisciplinary care team and community resources to integrate services.
• Facilitate waiver enrollment/disenrollment processes.
• Travel locally 25–40% as required.
