Molina Healthcare is seeking a TX-licensed LVN/LBSW/LMSW to join our LTSS Team as a Case Manager serving Medicaid members in the southern Dallas County area. The role involves in-home assessments, care planning, and coordinating services with a multidisciplinary team. Travel to member homes is expected; mileage is reimbursed.

Qualifikationen

• 2+ years health care experience; LTSS preferred.
• Active driver’s license and reliable transportation.
• Proficient in MS Office (Outlook, Excel, Teams).
• Ability to travel 25–40% locally.

Aufgaben

• Complete member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and other health professionals.
• Monitor and adjust care plans based on progress and barriers.
• Coordinate services across LTSS, behavioral health and home/community resources.
• Assess medical necessity and authorize waiver services.
