Job summary

Molina Healthcare seeks a care coordination professional to support LTSS and care management across settings. You will collaborate with a multidisciplinary team to ensure member progress toward outcomes and to integrate services across the continuum.

Responsibilities include comprehensive assessments, care plan development, monitoring, and coordinating waiver enrollment while addressing barriers to care. Travel up to 40% may be required.

Qualifications

• 2+ years health care experience including LTSS or care management.
• Licensure/certification required only if mandated by state contract.
• Active and unrestricted license if licensed; valid driver's license.
• Knowledge of community resources and ability to work with diverse populations.
• Strong verbal and written communication skills and self-motivation.

Responsibilities

• Provide care management support for LTSS and coordinate with multidisciplinary teams.
• Complete comprehensive member assessments within timelines and support waiver processes.
• Develop and monitor care plans, including waiver service plans with providers and families.
• Promote integration of behavioral health, LTSS, and community resources.
• Assess medical necessity, authorize services, and coordinate ICT meetings.
• Travel 25-40% as required by state contracts.
