Job summary

Molina Healthcare is seeking a Care Coordinator to support LTSS care management and care coordination across settings. You will assess members, develop care plans, and coordinate services with a multidisciplinary team to improve outcomes and manage costs.

Travel up to 25-40% locally may be required. The role emphasizes proactive communication, documentation, and navigation of community resources to support at-risk members.

Qualifications

• At least 2 years health care experience with LTSS.
• Clinical licensure/certification may be required by state contract.
• A bachelor's degree in a health care related field may be required by state.
• Valid and unrestricted driver’s license and transportation for travel.
• Strong knowledge of community resources and ability to work with diverse populations.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, and health professionals.
• Monitors care plan effectiveness and documents progress toward goals.
• Promotes integration of services across LTSS and behavioral health networks.
• Assesses medical necessity and authorizes waiver services.
• Collaborates with ICT meetings for approvals/denials.
• Interacts with members to address barriers to care and supports change.
