Job summary

Qualifications

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members, caregivers, physicians and providers.
• Promote integration of services across behavioral health and LTSS.
• Identify barriers and coordinate resources to support members.
