Responsibilities include comprehensive assessments, waiver processes, care plan development, and ongoing monitoring. Travel up to 40% based on state contracts and regulatory requirements, with emphasis on collaboration and quality outcomes.

Qualifications

• Requires 2+ years health care experience, LTSS exposure preferred.
• License/certification may be required by state contract.
• Bachelor’s degree may be required by state/contract.
• Valid driver’s license and reliable transportation.
• Experience with diverse populations and caring for individuals with disabilities.

Responsibilities

• Complete assessments within regulated timelines, including home visits when needed.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with the care team and family.
• Monitor and adjust care plans based on outcomes.
• Coordinate LTSS and behavioral health services across settings.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and funding sources.
• Lead ICT meetings for service approvals/denials.
• Educate and motivate members using motivational interviewing.
• Identify barriers and coordinate resources to address needs.
• Flag critical incidents and develop prevention plans.
• Collaborate with licensed care managers as required.
• Travel 25-40% as contracted.
