Job summary

Molina Healthcare seeks a Care Coordination Specialist to support LTSS members by collaborating with an interdisciplinary team to deliver integrated care. You will assess needs, develop and monitor care plans, and ensure adherence to program requirements.

Responsibilities include managing waiver processes, coordinating with physicians and caregivers, and ensuring timely member progress toward outcomes while navigating diverse care settings. Some travel required.

Qualifications

• 2+ years of health care experience, LTSS exposure preferred.
• Clinical licensure/certification required only if state contract/regulation dictates.
• Bachelor's degree in health care field may be required per state.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and document outcomes.
• Promote service integration across behavioral health and LTSS.
