Job summary

Molina Healthcare is seeking a Care Manager to support integrated care delivery for members with high-need LTSS needs in the United States. You will coordinate assessments, develop care plans with families and providers, and ensure timely services while tracking progress toward outcomes.

Qualifications

• 2+ years health care experience including LTSS
• Care management experience or equivalent
• Bachelor's degree may be required in some states

Responsibilities

• Complete comprehensive member assessments within required timelines.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers and providers.
• Monitor care plan effectiveness and document outcomes.
• Promote integrated care across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Collaborate in interdisciplinary care team meetings for approvals.
• Travel 25-40% as required.
