Job summary

Molina Healthcare is seeking a care management professional to coordinate long-term services and supports (LTSS) for high-need members. You will work with a multidisciplinary team to ensure timely assessments, care planning, and seamless service delivery across settings.

Responsibilities include waiver processes, care plan development, and monitoring outcomes with attention to psycho-social and medical barriers. Significant travel (25-40%) may be required to meet member needs.

Qualifications

• 2+ years in health care with 1+ year in care management or LTSS.
• Active and unrestricted license in state of practice.
• In some states, a health-related Bachelor's degree may be required.
• Valid driver's license and reliable transportation for travel.
• Strong communication and time-management abilities.
• Experience with substance use disorders a plus.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits when required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers and providers.
• Monitors and documents care plan effectiveness and progress toward goals.
• Promotes LTSS and behavioral health integration to ensure continuity of care.
• Coordinates ICT meetings for authorization decisions and team collaboration.
• Travel 25-40% as required by state or contract.
