Job summary

Molina Healthcare is seeking a skilled care coordinator to support LTSS members. You will partner with a multidisciplinary team to coordinate services across the continuum, conduct comprehensive assessments, and develop individualized care plans that drive outcomes and cost-effective care.

Qualifications

• At least 2 years of health care experience, incl. LTSS and care management.
• Clinical licensure/certification required only if mandated by state contract.
• In some states, a health care-related bachelor's degree may be required.
• Valid driver's license, reliable transportation for travel requirements.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Develop and implement care plans and waiver service plans with care team and members.
• Monitor care plan effectiveness and document interventions and outcomes.
• Promote service integration across behavioral health, LTSS, and community resources.
• Assess medical necessity and authorize waiver services; coordinate ICT meetings.
