Job summary

UnitedHealth Group is seeking a case manager to support LTSS members through intake, assessment, care planning, and ongoing case management. You will document activities, monitor services, and coordinate with CMA agencies while traveling within assigned counties.

This role requires confidentiality, strong communication, and the ability to conduct face‑to‑face visits, with travel up to 50% in Colorado. Competitive hourly pay: $27–$40, plus comprehensive benefits, incentive programs, and a 401k.

Qualifications

• Bachelor's degree in human services, nursing, social work, psychology or public health.
• 5+ years of LTSS experience or an education/experience mix.
• MS Office proficiency (Word, Excel, Outlook).

Responsibilities

• Intake, screening, referrals, assessments, and development of support plans.
• Ongoing case management, documentation, and monitoring of member health and welfare.
• Maintain confidentiality per policy and sign-off in the employee handbook.
• Conduct quarterly or more frequent contact and semi-annual face-to-face visits.
• Complete annual reassessments and update care plans in the system.
• Monitor service delivery within the approved plan and PAR.
• Attend CMA training and internal staff development sessions.
• Travel within assigned territory; drive or arrange transport as needed.
