Job summary

Qualifications

• Bachelor’s degree in a human behavioral science field or equivalent experience.
• 5+ years of LTSS experience or combination of education and experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Driver’s license and reliable transportation; travel up to 50% within territory.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans and ongoing case management.
• Monitor member health and welfare; document contacts and activities in the prescribed system.
• Conduct quarterly and annual face-to-face assessments; update assessments and care plans as needed.
• Ensure confidentiality and compliance with CMA contract requirements; participate in training.
