Job summary

Molina Healthcare, Inc. is seeking a Care Manager to support LTSS-focused care coordination across multiple Ohio counties.

You will work with a multidisciplinary team to develop and monitor care plans, coordinate waiver services, and address barriers to care through in-person visits and community resources. The role requires travel, strong communication, and the ability to coordinate with physicians, caregivers, and care teams in diverse settings.

Qualifications

• 2+ years of health care experience, including LTSS exposure and care management experience.
• Licensure/certification required only if state contract/regulation requires it; active and unrestricted in state of practice if licensed.
• In some states, a bachelor's degree in a health care related field may be required.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job travel.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with member, caregivers, physicians, and care network.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Assesses medical necessity and authorizes waiver services as appropriate.
• Evaluates benefits and advises on funding sources.
• Facilitates ICT meetings for approval/denial of services and collaboration.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Assesses barriers to care and provides coordination to address psycho/social, financial, and medical obstacles.
