Responsibilities

• Assesses member needs, coordinates LTSS care plans and waiver services.
• Completes member assessments within timelines, including home visits.
• Facilitates waiver enrollment/disenrollment processes and ICT meetings.
• Monitors care plan effectiveness and documents interventions.
