Job summary

Molina Healthcare, Inc. in Ohio is seeking a Care Manager to support LTSS care coordination and collaboration with a multidisciplinary team to deliver high-quality, cost-effective member care.

You will help coordinate services, assess needs, and guide members toward outcomes. Travel up to 40% locally, strong communication, care planning, and knowledge of community resources are essential as you work with members, caregivers, physicians and care teams across settings to ensure timely, appropriate

Qualifications

• 2+ years health care experience, including LTSS.
• Licensure required only if state contract requires it; active/unrestricted if applicable.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong verbal and written communication.
• Experience with diverse populations.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and other health care professionals.
• Monitor and evaluate care plan effectiveness and adjust as needed.
• Promote integration of services across LTSS and behavioral health.
• Assess medical necessity and authorize appropriate waiver services.
• Facilitate interdisciplinary care team meetings for approvals/denials.
• Travel 25-40% locally as required.
