Job summary

Molina Healthcare is seeking a dedicated care coordination professional in Ohio to support LTSS and waiver services across multiple counties. You will collaborate with multidisciplinary teams to assess needs, develop care plans, and monitor progress toward outcomes in diverse settings.

Responsibilities include conducting home visits, managing waiver enrollment, promoting service integration, and coordinating with providers.

Qualifications

• At least 2 years of health care experience, including at least 1 year with persons with disabilities/chronic conditions LTSS, and 1 year in care management or equivalent.
• Clinical licensure/certification required only if mandated by state contract or regulations; active and unrestricted if licensed.
• Some states may require a bachelor’s degree in a health care field.
• Valid driver's license, transportation, and adequate auto insurance for travel.
• Knowledge of community resources.
• Ability to work across diverse settings and with diverse populations.
• Ability to work independently with minimal supervision and manage multiple projects.
• Strong verbal and written communication skills.
• Microsoft Office proficiency and comfort with online portals/databases.
• In some states, at least one year of experience working directly with individuals with substance use disorders.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians, and other health professionals.
• Monitors care plan effectiveness, documents interventions and outcomes, and suggests changes.
• Promotes integration of behavioral health care and LTSS with community resources to improve continuity of care.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and advises on funding sources.
• Facilitates interdisciplinary care team meetings for approval/denial of services.
• Identifies barriers to care and provides coordination to address psycho/social, financial, and medical obstacles.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers/leadership as needed.
• Travel 25-40% locally as required.
