Job summary

Qualifications

• 2+ years health care experience, incl. 1 year LTSS/care management or equivalent
• License/certification required only if state contract mandates it
• Ability to work independently with minimal supervision
• Valid driver's license and reliable transportation for travel requirements

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, and providers.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of LTSS with behavioral health and community resources.
• Assess medical necessity and authorize waiver services.
• Collaborate with interdisciplinary care team for approvals/denials.
• Provide education and motivation during member contacts.
• Identify barriers to care and assist with psycho-social, financial, and medical obstacles.
• Travel 25-40% as required by state/contractual needs.
