Job summary

Molina Healthcare, Inc. is seeking a Care Coordination Specialist to support LTSS members across multiple counties in Ohio.

You will work with a multidisciplinary team to assess needs, develop care plans, and coordinate waiver services, while promoting integrated care and cost-effective outcomes. The role involves in-person visits, travel up to 40%, and collaboration with physicians, caregivers and support networks to improve member health and access to resources.

Qualifications

• 2+ years health care experience including LTSS or care management.
• Valid driver's license and reliable transportation.
• Ability to work independently and manage multiple projects.
• Proficiency with Microsoft Office and online databases.
• Experience with individuals with substance use disorders in some states.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor and evaluate care plan effectiveness.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and funding sources.
• Facilitate ICT meetings for approval/denial of services.
• Educate and motivate members using motivational interviewing and Molina guidelines.
• Identify barriers to care and assist with psycho/social and medical obstacles.
• Collaborate with licensed care managers as needed.
• Travel 25-40% local may be required.
