You will work with a multidisciplinary team to develop and monitor care plans, facilitate waiver processes, and promote integrated services for high-need members. Responsibilities include timely assessments, enrollment/disenrollment, and collaboration with healthcare professionals to address medical, behavioral health, and social needs.

Qualifikationen

• 2+ years healthcare experience with LTSS and care management.
• Valid driver's license and reliable transportation.
• Ability to work independently and manage multiple projects.
• Proficiency with Microsoft Office and online portals.
• Experience with substance use disorder populations (state dependent).

Aufgaben

• Complete comprehensive member assessments within timelines and, when required, conduct in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members, caregivers, physicians, and team.
• Promote integration of services across LTSS, behavioral health, and community resources.
• Travel locally 25-40% as required by contracts.
