Qualifications

• 2+ years of health care experience including LTSS
• Active license if required by state contract/regulation
• Bachelor's degree may be required in some states
• Valid unrestricted driver's license and reliable transportation

Responsibilities

• Perform comprehensive member assessments within timelines, including home visits
• Coordinate waiver enrollment and disenrollment
• Develop and implement care plans with members, caregivers and providers
• Monitor care plans and adjust as needed
• Promote integration of LTSS with behavioral health and community resources
• Assess medical necessity and authorize waiver services
• Facilitate ICT meetings for service approvals/denials
