Molina Healthcare is hiring for a care management role focused on long-term services and supports (LTSS). You will coordinate care across a multidisciplinary team, complete assessments, and facilitate waiver processes.

Travel will be 25-40% to support members in several counties across Ohio. The role emphasizes collaboration with physicians, caregivers, and other health professionals to address medical, psycho-social, and financial barriers to care, aiming to achieve desired member outcomes with

Qualifications

• At least 2 years of health care experience, including 1 year with LTSS and 1 year in care management.
• Valid and unrestricted driver’s license with reliable transportation and adequate auto insurance.
• Experience working with diverse populations and strong communication skills.

Responsabilités

• Completes comprehensive member assessments within timelines, including in-person home visits.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members and care network.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of behavioral health and LTSS with community resources.
• Assesses medical necessity and authorizes waiver services.
