Job summary

Centene is seeking a Care Manager in Illinois to assess member needs, coordinate services with providers, and develop long‑term care plans for aging and disabled waivers. The hybrid-remote role includes 75% travel to member homes.

Initial training runs 8-5 pm CT, then 8-4:30 pm CT with a 30‑minute lunch. You will educate members and families about services, monitor outcomes, and collaborate with care teams to ensure access to care.

Qualifications

• Requires Bachelor's degree and 1 year of related experience.
• RN licensed in Illinois or LPN with 1 year experience in elderly care.
• Bachelor or Master’s Degree in human services related field as listed.

Responsibilities

• Evaluates member needs, resources, and recommends the care plan.
• Develops long‑term care/service plans and identifies providers/resources.
• Coordinates between member/family and care team to ensure access to services.
• Provides resource support for employment, housing, and other needs based on assessments.
• Monitors care plans and outcomes and makes recommendations.
• Interacts with long‑term care providers to meet member needs.
• Collects and maintains member information for compliance.
• May perform on‑site visits to assess needs and collaborate with providers.
• Educates members and families on procedures, services, and benefits.
• Provides feedback to leadership to improve quality and efficiency.
