Job summary

Centene is seeking a LTSS Service Care Coordinator in Springfield, IL to develop and coordinate holistic care plans for long-term care members. The role combines remote work with about 50% local travel for in-home visits, primarily serving individuals on LIHT or aging waivers.

Ideal candidates bring case management, advocacy, or home visit experience, and may qualify under Illinois plan criteria including RN license or human services degrees.

Qualifications

• Bachelor's degree plus 1 year related experience.
• RN license in Illinois or Bachelor/Master’s in human services as per Illinois plan.
• LPN with 1 year experience in comprehensive assessments and elderly care.

Responsibilities

• Evaluate member needs, resources, and plan the best outcomes.
• Develop and update long-term care/service plans.
• Coordinate between member/family and care team to ensure services are accessible.
• Provide resource support on employment, housing, independence, and community services.
• Monitor and adjust care plans and member status based on needs and outcomes.
• Engage with healthcare providers and partners to meet member needs.
• Collect and maintain member information to ensure regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Educate members and families on procedures, options, and benefits.
• Provide leadership with feedback to improve quality and efficiency of care.
