Job summary

Wellpath is seeking an LPN/LVN to provide compassionate, patient-centered care in a clinical setting. Under supervision, you will perform nursing tasks, administer medications, and contribute to care planning for underserved patients.

Qualifications

• Must be licensed as LPN/LVN in the state of employment.
• CPR certification required.
• Prefer a minimum of 1 year clinic experience.
• Graduate from accredited nursing program.

Responsibilities

• Under RN supervision, assess patients' physical, psychological, and social needs to inform care planning.
• Administer medications as prescribed, including IV therapy if certified.
• Maintain accurate nursing documentation and participate in shifts and handoffs.
• Attend mandatory staff meetings and trainings to ensure adherence to policies.
