Job summary

Wellpath / Correct Care Solutions seeks a Licensed Practical Nurse (LPN) to contribute to patient care under RN supervision in a clinic-based health facility. The role emphasizes assessment, medication administration (including IV therapy where certified), and coordinated nursing care in a facility setting.

Requirements include graduation from an accredited nursing program, active LPN/LVN licensure in the state, CPR certification, and at least one year of clinic experience.

Qualifications

• Graduation from an accredited school of nursing.
• Minimum one year clinic experience preferred.
• Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse in the state of employment.
• Must be able to obtain and maintain CPR certification.

Responsibilities

• Under RN supervision, assess patients’ physical, psychological, and social dimensions to contribute to treatment planning.
• Administer medications, including IV therapy where certified, and perform diagnostic tests per protocols.
• Assist with controlled substance counts and respond to codes or health emergencies.
• Document nursing encounters using SOAP charting and communicate with the care team.
• Attend mandatory staff meetings and training sessions to maintain professional development.
