Wellpath is seeking a Licensed Practical Nurse (LPN) to join our compassionate team serving underserved and vulnerable patients in Colorado. You will operate under RN supervision, providing patient education, administering medications, and supporting treatment plans in a supportive, team-driven environment.

Formación

• Graduate from an accredited school of nursing.
• Minimum of one year clinic experience preferred.
• Current LPN/LVN license in the state of employment and CPR certification.

Responsabilidades

• Under RN supervision, assesses patients' physical, psychological, and social needs and contributes to individualized treatment planning.
• Administers medications, including IV therapy (if certified), and performs diagnostic tests per protocol.
• Collaborates in controlled substance counts and adheres to safety procedures during shifts.
• Assists in procedures, documents nursing encounters, and communicates effectively with the care team.
• Attends mandatory meetings and ongoing training to maintain high-quality patient care.
