Job summary

Elara Caring in Kirksville, Missouri, is hiring a Licensed Practical Nurse LPN to provide high-quality in-home care across Moberly, MO and surrounding areas. This part-time, hourly role offers flexible hours and the chance to make a difference in patients' lives by delivering care plans in their home settings.

You will work with a clinical team, review care plans, manage medications, educate families, and help prevent hospitalizations while meeting health and regulatory requirements.

Qualifications

• Current LPN/LVN license in Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of acute care experience; home health or hospice preferred.
• Strong communication and interpersonal skills.
• Ability to manage time effectively and stay organized in the field.
• Knowledge of healthcare regulations and patient care standards.
• Reliable transportation with travel ~50%.
• Ability to meet health screening and compliance requirements.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.
