Job summary

Elara Caring in Jefferson City, Missouri is urgently hiring a Part-Time Licensed Practical Nurse LPN to support in-home care across the community. Join a mission to provide high-quality care where patients live and to make a meaningful difference in their health journeys.

Deliver in-home nursing care per patient plans, review Plan of Care before visits, communicate updates, assist with medication management, educate patients and families, identify safety concerns, and collaborate with RN Case

Qualifications

• Current LPN/LVN license in Missouri.
• Graduate of an accredited nursing program.
• Minimum 1 year of acute care experience; home health or hospice preferred.

As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:

What is required?

• Current LPN/LVN license, in the state of Missouri


• Graduate of an accredited nursing program


• At least 1 year of experience in acute care ( home care, home health or hospice preferred)


• Strong communication and interpersonal skills


• Ability to manage time effectively and stay organized in the field


• Knowledge of healthcare regulations and patient care standards


• Reliable personal transportation (travel required ~50%)


• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
