Job summary

Elara Caring in Missouri is hiring a Licensed Practical Nurse (LPN) to join our Personal Care Services team on a part-time, hourly basis. You will deliver in-home nursing care, follow patient care plans, and communicate updates to the care team.

Travel is required about 50% as you visit patients in their homes. The role reports to the Clinical Manager or Branch Director and offers opportunities for advancement, tuition reimbursement, and a competitive compensation package.

Qualifications

• Current LPN/LVN license in Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of acute care/home health or hospice experience preferred.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.

What is required?

• Current LPN/LVN license, in the state of Missouri
• Graduate of an accredited nursing program
• At least 1 year of experience in acute care ( home care, home health or hospice preferred)
• Strong communication and interpersonal skills
• Ability to manage time effectively and stay organized in the field
• Knowledge of healthcare regulations and patient care standards
• Reliable personal transportation (travel required ~50%)
• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
