Elara Caring is hiring a part-time Licensed Practical Nurse (LPN) for Personal Care Services in the Arbor North Jackson, Michigan area. The role offers an hourly wage of $34–$36 and requires about 50% travel to patient homes, delivering in-home nursing care per care plans.

You will work under the Clinical Manager or Branch Director for Personal Care Services, with opportunities for professional growth, tuition reimbursement, and a supportive team environment.

Qualifications

• Must hold current LPN/LVN license in the state of Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of experience in acute care (home care, home health or hospice preferred).

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.

As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:

What is required?

• Current LPN/LVN license, in the state of Missouri
• Graduate of an accredited nursing program
• At least 1 year of experience in acute care ( home care, home health or hospice preferred)
• Strong communication and interpersonal skills
• Ability to manage time effectively and stay organized in the field
• Knowledge of healthcare regulations and patient care standards
• Compassionate, patient-first mindset
• Reliable personal transportation (travel required ~50%)
• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
