Job summary

Elara Caring in Boston is seeking a Licensed Practical Nurse (LPN) to join our in-home care team. You will deliver nursing care per patient plans, coordinate visits, and collaborate with care teams to support patients in their homes.

The role requires an active LPN license and 1 year of acute care experience (home health or hospice preferred). Strong communication, time management, and a compassionate, patient-first mindset are essential. Travel ~50% is expected.

Qualifications

• Current LPN/LVN license in the state of Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of experience in acute care (home care, home health or hospice preferred).
• Strong communication and interpersonal skills.
• Ability to manage time effectively and stay organized in the field.
• Knowledge of healthcare regulations and patient care standards.
• Compassionate, patient-first mindset.
• Reliable personal transportation (travel required ~50%).
• Ability to meet health screening and compliance requirements.

Responsibilities

You will report to the Clinical Manager or Branch Director for Personal Care Services.
