Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) in Jefferson City, Missouri for part-time hourly work in Personal Care Services. The role focuses on delivering in-home nursing care, reviewing care plans, and communicating patient updates to the care team.

The LPN will educate patients and families, assist with medication management, and collaborate with RNs and physicians to prevent hospitalizations. Travel approx. 50% is required.

Qualifications

• Current LPN/LVN license in Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of acute care experience (home care, home health, or hospice preferred).
• Strong communication and interpersonal skills.
• Ability to manage time effectively in the field.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.

As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:

What is required?

• Current LPN/LVN license, in the state of Missouri
• Graduate of an accredited nursing program
• At least 1 year of experience in acute care ( home care, home health or hospice preferred)
• Strong communication and interpersonal skills
• Ability to manage time effectively and stay organized in the field
• Knowledge of healthcare regulations and patient care standards
• Reliable personal transportation (travel required ~50%)
• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
