Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) to provide high-quality in-home care in Jefferson City, MO and surrounding areas. The role focuses on delivering care per patient plans, monitoring progress, and coordinating with the care team to maintain comfort and safety in the home setting.

Responsibilities include reviewing plans before visits, documenting updates, managing medications, and educating patients and families on self-care.

Qualifications

• Current LPN/LVN license in Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of acute care, home care, or hospice experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.

As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:

What is required?

• Current LPN/LVN license, in the state of Missouri
• Graduate of an accredited nursing program
• At least 1 year of experience in acute care (home care, home health or hospice preferred)
• Strong communication and interpersonal skills
• Ability to manage time effectively and stay organized in the field
• Knowledge of healthcare regulations and patient care standards
• Reliable personal transportation (travel required ~50%)
• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
