Elara Caring is hiring Licensed Practical Nurse LPNs to deliver in-home nursing care across Missouri. You’ll follow patient care plans, coordinate with care teams, and travel to patients’ homes to provide Right Care at the Right Time.

Qualifications

• Current LPN/LVN license in Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of experience in acute care (home care, home health or hospice preferred).
• Strong communication and interpersonal skills.
• Ability to manage time effectively and stay organized in the field.
• Knowledge of healthcare regulations and patient care standards.
• Compassionate, patient-first mindset.
• Reliable personal transportation with travel required ~50%.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.

What is required?

You will report to the Clinical Manager or Branch Director for Personal Care Services.
