Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) to deliver high‑quality in‑home nursing care in Jefferson City and surrounding areas. This part‑time hourly role reports to the Clinical Manager and Branch Director for Personal Care Services.

You will perform tasks in patients’ homes, review plans of care before visits, document updates, and educate families to support self‑care and safety. Expect travel about 50% as part of caring for patients in their home settings.

Qualifications

• Current LPN/LVN license in Missouri
• Graduate of an accredited nursing program
• At least 1 year of experience in acute care (home health or hospice preferred)

Responsibilities

• Deliver in-home nursing care aligned with patient care plans
• Review and follow Plan of Care prior to each visit
• Communicate patient updates, concerns, and changes in condition
• Assist with medication management and documentation
• Educate patients and families on self-care techniques
• Identify safety concerns and help prevent hospitalizations
• Collaborate with RN Case Managers, physicians, and care team

What is required?

• Current LPN/LVN license, in the state of Missouri
• Graduate of an accredited nursing program
• At least 1 year of experience in acute care ( home care, home health or hospice preferred)
• Strong communication and interpersonal skills
• Ability to manage time effectively and stay organized in the field
• Knowledge of healthcare regulations and patient care standards
• Reliable personal transportation (travel required ~50%)
• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
