Job summary

Hudson Regional Health is seeking an LPN to coordinate the day-to-day Patient Program, ensuring efficient flow and a positive patient experience. The role serves as a liaison among patients, physicians, clinical staff, and the program manager to facilitate communication and coordinated care.

Qualifications

• Active New Jersey LPN license required.
• Graduate of an accredited practical nursing program.
• Minimum 2 years of clinical nursing experience; outpatient or ambulatory care preferred.
• Experience with patient care coordination, patient flow, scheduling, or clinical operations.
• Proficiency with EHR systems and standard computer applications.
• Excellent organizational, communication, and interpersonal skills.
• Ability to prioritize multiple tasks, resolve routine issues, and escalate concerns as needed.
• Demonstrated ability to work with patients, families, physicians, nurses, and multidisciplinary teams.

Responsibilities

• Oversee daily patient flow and coordinate movement through the program to support timely and efficient care.
• Serve as primary liaison between patients, physicians, clinical staff, and the program manager.
• Facilitate communication regarding patient needs, appointments, scheduling, and program-related concerns.
• Support physicians and clinical staff with the coordination of daily program activities and patient needs.
• Assist the program manager with day‑to‑day operations, workflow, scheduling, and administrative functions.
• Monitor program activities and identify operational issues that may affect patient flow or the patient experience.
• Collaborate with internal departments and external providers as needed to ensure continuity and efficiency of services.
• Maintain accurate patient and program documentation in accordance with organizational policies and regulatory requirements.
• Provide patients and families with information regarding program services, processes, and resources.
• Assist with resolving patient concerns and elevate clinical or operational issues appropriately.
• Collaborate with the clinical team to ensure patient needs are communicated and addressed promptly.
• Support the implementation of program policies, procedures, quality initiatives, and process improvements.
• Maintain confidentiality and comply with HIPAA and applicable regulations.
