People Care in Brooklyn seeks an experienced LPN coordinator to manage patient cases end-to-end, schedule nurse visits, and oversee service-hour reviews. You will serve as the liaison among patients, families, physicians, field staff and agency teams; work 100% on-site at our Brooklyn office.

Ideal candidates hold a current NYS LPN license in good standing, have home care or LHCSA scheduling experience, and demonstrate strong communication, organization, and EMR fluency to ensure timely,

Qualifications

• Current NYS LPN license in good standing.
• Experience in home care, case management, or scheduling LHCSA setting preferred.
• Strong care coordination workflow knowledge and service authorization.

Responsibilities

• Case coordination from end to end (care plans, service needs, condition changes).
• Nurse visit scheduling based on patient need, authorization, and staff availability.
• Review service hours and eligibility to support care planning and authorized services.
