Job summary

315 Genesee Region Home Care Association Inc is seeking an LPN-level professional to coordinate patient services under the direction of the team manager/designee in Rochester. The role prioritizes daily patient schedules, arranges aide, therapy, social work and interpreter services, and ensures timely referrals are assigned.

Qualifications

Responsibilities

• Validates supplemental orders and processes per policy.
• Assists in other duties deemed appropriate to the LPN scope of practice.
• Manages multiple phone calls with timely follow-up to meet callers’ needs.
• Provides field staff assistance and resources for day-to-day issues under team manager guidance.
• Checks email promptly and follows up to attend sender’s needs.
• Collects, organizes and reviews information for reporting and initiates follow-up.
• Updates the EMR per agency standards.
• Performs transfers or discharges as required by regulation.
• Builds and maintains strong, communicative relationships with the team.
• Provides exemplary customer service.
• Performs general office duties including phones, documentation, filing, mailing, and supplies.
