Job summary

CenterWell Home Health is seeking a Patient Services Coordinator, LPN/LVN to serve as a vital link between clinical staff and patients, coordinating scheduling and all aspects of care. The role requires an active RN/LPN license in the practicing state and at least one year of Home Health experience.

Responsibilities include managing patient schedules, discharge visits, OASIS collections, and coordinating with the Branch Director.

Qualifications

• Licensed RN or LVN in the state of practice.
• Minimum 1 year of Home Health experience.
• Ability to coordinate scheduling and patient care.

Responsibilities

• Manage schedules for all patients, including supervisory visits.
• Schedule discharge visits, OASIS Collection, recert visits, and Transfer to Inpatient Facility (TIF) OASIS collection visits.
• Reschedule declined or missed visits and process reassigned and rescheduled visits.
• Prepare and edit weekly agent schedules, review productivity/geographic issues, and forward for approval.
• Assist with weekly case conferences.
• Receive calls from field staff and refer clinical questions to the Branch Director as necessary.
• May perform patient visits and/or participate in on-call rotation.
• Maintain the client hospitalization log, including entering coordination notes.
• Send electronic logs to all office, field, and sales staff; may include lab reports.
• Verify visit paper notes in the scheduling console as needed.
• Run all scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
• Assist with internal transfer of patients between branch offices.
• Additional duties may vary by branch census and are subject to local leadership.
